Anyone wishing to register with the practice must live within an 8 mile radius of the surgery.

If you wish to register with Toome Surgery, please come to the surgery to collect the correct forms to complete. 

Each new patient will require a registration medical appointment with The Practice Nurse which will be arranged for you, when the registration process is complete.

To register you will need to provide

   .    Photographic ID

   .    Proof of address 

Please be prepared to give us a brief medical history and medication list.

This can be printed out from your previous GP.

It takes approximately 6 weeks for medical notes to be transferred from previous GP to BSO and then onto us.

Once your registration is complete, you will be sent out a new patient medical card from BSO.

REGISTERING A NEWBORN AT THE PRACTICE

Following the registration of a newborn baby at your local Council Office, you will be issued with a yellow registration form. Please bring this form to the practice and we will register your baby.
